Dangerous Goods Declaration Forms

The Dangerous Goods Declaration form is required for all international shipments of dangerous goods/hazardous materials by air. This Dangerous Goods Declaration form includes air certification and an area for the 24 hour emergency response phone number. The form includes the basic description sequence (UN number, proper shipping name, hazard class, packing group), as well as the declaration statement for air eligibility.

The Dangerous Goods Declaration form are in laser format. The laser forms are available in 8 1/2" x 11", single sheet, with columns and are packed 100 sheets/package.
